With WotLK Classic Patch 3.4.4 Fall of the Lich King new dungeon difficulty arrived called Titan Rune Gamma Dungeons or Heroic+ dungeons how it is called by community. Inside of each Heroic Dungeon, close to the entrance is a Mysterious Device, an object which players can interact with. To activate this difficulty, players need to interact with the Mysterious Device inside of each Heroic dungeon and channel it until the Defense Protocol Gamma activates, drastically increasing difficulty and rewards in a dungeon. There are 13 Dungeons in WotLK available to be completed in Heroic+ mode plus 3 new dungeons added in Phase 4 which have the same difficulty and share the same loot on Heroic mode: The Forge of Souls, The Pit of Saron and Halls of Reflection. All dungeons are located in new Northrend zones:

- Utgarde Keep and Utgarde Pinnacle located in Howling Fjord zone;
- Azjol-Nerub and Ahn'kahet: The Old Kingdom located in Dragonblight zone;
- Drak'Tharon Keep and Gundrak located in Zul'Drak zone;
- The Violet Hold located in Dalaran;
- Halls of Stone and Halls of Lightning located in The Storm Peaks zone;
- The Culling of Stratholme located in Caverns of Time in Tanaris zone;
- Trial of the Champion located in Icecrown zone;
- The Forge of Souls, Pit of Saron and Halls of Reflection also located in Icecrown zone.
